When Honesty Costs You
Your Care

Cannabis, psychiatric medication, and the gap between what the evidence recommends and what too many patients experience in the room.

You booked the appointment. You showed up. Maybe it took you a while to get here, maybe it did not, but either way you were ready to be honest with your provider. And somewhere in that intake, you mentioned cannabis.

The room changed.

It is harmful, they said. I cannot prescribe while you are using. Come back when you have stopped. Some version of that, delivered with the kind of finality that did not leave room for your specific situation, your history, or the years you have been stable on medication that was actually working. You walked out without a prescription, or without the help you came in asking for, confused in a way that is hard to articulate because you did not do anything wrong.

On the drive home, the spiral. If this one says no, they probably all say no. And you know what happens when you miss a few days of your medication. You have felt it. So you make a quiet decision: you will not bring this up again. You will manage it alone, because honesty just cost you your care.

What happened in that room has a name in the research literature. It is not standard care. And you deserved better than what you got.

The Brain Is an Organ. Treat It Like One.

We do not tell a cardiologist's patient to stop eating salt before treating their heart failure. We do not tell a gastroenterologist's patient their Crohn's disease will wait until their diet is perfect. We treat the organ. We factor in the variables. We stay in the room.

The brain is an organ. Depression is not a character flaw. ADHD is not a lack of willpower. Bipolar disorder is not a mood preference. These are conditions of a biological system, and psychiatry is the medicine that treats them. When that medicine is withheld not because of a clinical finding but because of a lifestyle disclosure, we have stopped treating the organ and started judging the person.

There is a parallel worth naming. Research has shown repeatedly that no-harm contracts signed by suicidal patients have no clinical efficacy. They do not reduce suicidal behavior. What they do is reduce the therapist's anxiety. The contract is for the clinician, not the client. A cannabis refusal operates the same way.

What the Research Has Named

When one piece of information about a patient comes to dominate the entire clinical picture, medical researchers call it diagnostic overshadowing. The concept was first documented in patients with intellectual disabilities, whose new or unrelated symptoms were routinely attributed to their existing diagnosis and left untreated. It has since been applied across medicine, including to patients with substance use histories, where it functions the same way: the substance becomes the story, and the person's actual clinical needs become secondary.

The American Academy of Family Physicians identified it in 2023 as "probably the type of medical underuse that patients fear most," pointing to clinician bias as a primary driver (Lazris et al., 2023). A related pattern documented in Frontiers in Psychiatry is called therapeutic pessimism, in which providers develop diminished treatment plans not because the evidence calls for it, but because they have already decided, before a thorough assessment, that this patient is unlikely to engage or improve (Cunningham et al., 2023). Not a clinical conclusion. A prejudgment, dressed as one.

A 2025 study by researchers at Yale School of Medicine found significant gaps in physician knowledge about cannabis, with more than half having never recommended it in any clinical context and a third reporting inadequate knowledge to guide those conversations. Their willingness to treat varied widely and was, the researchers concluded, not consistent with the current strength of evidence.

The Arguments Behind the Closed Door

These refusals rarely happen in silence. There are stated reasons. Some have a partial basis in research. None of them, when examined carefully, justify a blanket refusal for a stable patient.

"Cannabis increases psychosis risk."

This is real for specific groups: adolescents, daily heavy users, high-potency products, and individuals with a personal or family history of psychotic disorders. A 2020 systematic review confirmed these as the meaningful risk factors (van der Steur et al., 2020). It is not a uniform risk for every adult using cannabis occasionally. The appropriate response is individualized screening, not a blanket policy.

"They might misuse or divert their stimulants."

This concern is legitimate and studied. A 2025 systematic review found past-year misuse rates around 22% among those prescribed stimulants. But cannabis use alone is not the risk factor the literature identifies. The actual predictors are peer misuse exposure, amphetamine-based formulations, and having surplus medication. The American Academy of Child and Adolescent Psychiatry recommends heightened monitoring and long-acting formulations, not refusal.

"It interacts with psychiatric medications."

Some interactions are real and do warrant monitoring. Cannabis and stimulants together can increase cardiovascular strain. CBD affects enzyme pathways relevant to some antidepressants. A 2022 systematic review, however, found low probability of clinically significant interactions for most common psychiatric medications (Lopera et al., 2022). The medications with the most documented interactions are anticonvulsants, blood thinners, and immunosuppressants, not the SSRIs and stimulants most commonly involved in these refusals.

"Prescribing creates regulatory exposure for me."

This is the argument that rarely gets named directly, but it is often present. DEA scrutiny of controlled substance prescribing is real, and providers feel it. The protection comes from documented individualized clinical reasoning, not from refusal. That is also exactly what the standard of care requires.

The Risk Nobody Calculated

When a provider closes the door over cannabis disclosure, one risk gets all the attention. The other one does not.

Someone who has been on an antidepressant for years stops taking it overnight. Within days they may feel like their nervous system is short-circuiting. Discontinuation syndrome affects between 15% and 56% of patients who stop abruptly, with severity increasing the longer they have been on the medication (Fava et al., 2024). For someone on lithium, research by Tondo and Baldessarini found the risk of suicidal behavior rises dramatically in the months following abrupt discontinuation. For someone with ADHD, remove the stimulant and the condition returns within days, sometimes with a crash that includes profound depression and dysphoria as dopamine levels plummet.

The interaction risk between cannabis and most common psychiatric medications is low for most patients and manageable through standard monitoring (Lopera et al., 2022). The door closed to prevent a risk. The data suggests it created a larger one.

There Is a Name for That Decision Too

On Patient Abandonment

Most people have never heard the clinical and legal term for what happened to them. The American Psychiatric Association defined patient abandonment in 2024 as the unilateral termination of a physician-patient relationship without giving adequate notice and without ensuring the patient has the opportunity to obtain alternative care, particularly when that termination occurs at a time when the patient required continued medical attention.

A stable patient on long-term psychiatric medication who is denied a refill over cannabis disclosure, with no referral and no transition plan, fits that definition. The discontinuation research establishes that continued attention was medically required. Intent and impact are two different things.

A disclosure of cannabis use is clinical information. The evidence-based response is a thorough assessment, not a closed door.

SSRIs and non-controlled antidepressants

Psychiatric conditions are not character deficiencies dressed up in diagnostic language. They are biological. They are treatable. And the people who carry them deserve providers who walk into the room with the full weight of the evidence behind them, not a reflex shaped by stigma, incomplete training, or institutional convenience. The research has been clear for years. Every patient who leaves a psychiatric office without care because they were honest about cannabis use represents a failure not of that patient, but of a system that has not yet closed the distance between what it knows and what it does. That distance has a cost. It shows up in emergency rooms, in relapses, in the quiet decisions people make to never ask for help again. Closing it is not idealism. It is the standard of care.

Care that starts with the whole person.
